I understand and agree that all documents submitted as part of this application may be securely stored in digital format. ThreeLeven Staffing Solutions complies with all applicable federal and state regulations regarding recordkeeping and document retention.

✅ SMS Notifications Consent
I agree to receive SMS notifications from ThreeLeven Staffing Solutions. Standard messaging rates may apply.

✅ Uniform Policy & Payroll Deduction
I acknowledge that ThreeLeven provides a required company uniform (shirt) at a total cost of $22.00, which will be deducted in two equal payments of $11.00 from my first two paychecks.

✅ Compliance and Authorization
I understand that:
• My documents may be accessed only by authorized personnel for employment purposes.
• No information will be shared with third parties without my consent, unless legally required.
• My records will be securely disposed of at the end of the retention period.
• I can request a readable paper copy of my electronic records at any time.

✅ State and Federal Compliance
I acknowledge that all policies and procedures are designed to meet both federal and applicable state employment laws, including those governing 1099 contractors.

✅ Electronic Signature Consent
I agree that my electronic signature is legally binding and has the same validity as a handwritten signature.

✅ Media Release Authorization
I grant ThreeLeven Staffing Solutions permission to take and use my photograph for identification, credentialing, and internal purposes.

✅ Voluntary Background Check Consent
I understand that I may be subject to a background check and voluntarily consent to the verification of my employment eligibility and criminal history as required by applicable contracts or venues.
